Passports and documents


According to the law of civil registration, issuance or extension of a travel document will be approved only upon updating the personal status, otherwise the travel document will be issued with the validity of one year or extended by one year only.

Upon request of issuance or extension, you will be asked to fill in a form of personal details. You can download the form here, and send it back to us by mail (for more details, please check inside the form).

Registration form for an Israeli citizen abroad:
An Israeli citizen which stays in Shanghai for a period longer than 30 days, can download a form of registration, fill in his details and send it to the Consulate. The detials will be saved for future contact.

Various certificates issued by the Ministry of Interior in Israel:

All of the following certificates are issued by the Ministry of Interior in Israel and can be oredred by Consulate in your behalf: Personal status registry, Israeli birth certificate, change of personal status, death certiciate, exit/entry record certificate.

Police Clearance:


In order to apply for a No Criminal Record Document, please submit this form in person at the Consulate. Please come with your passport as well. Please inform in advance if you need the document to be notarized by Ministry of Foreign Affairs in Israel.
